Wrinkle treatment with botulinum toxin

Botulinum toxin type A is among the most thoroughly studied active substances in aesthetic medicine. This page explains in factual terms how the treatment of mimic wrinkles works, how an appointment proceeds and what matters for safety and tolerability — without promises, with the standard of evidence-oriented patient information.

Terminology

“Botox” or “botulinum toxin” — which is correct?

In everyday speech, people usually refer to “Botox”. The medically correct term is botulinum toxin type A — an active substance used in medicine for decades. “Botox” is strictly speaking the brand name of a single preparation and is used colloquially for the whole substance group, similar to how “Kleenex” is used for facial tissues.

Throughout this information we consistently use the medically correct term botulinum toxin type A. The practice exclusively uses original preparations approved in Germany; the choice of preparation depends on the specific indication and the area to be treated.

In short

“Botox” and “botulinum toxin” mean the same thing in patient conversation. The difference lies only between brand name and substance name.

Mechanism of action

How botulinum toxin type A works

Mimic wrinkles — such as between the eyebrows, on the forehead or at the side of the eyes — arise from the repeated tension of certain facial muscles. Botulinum toxin type A temporarily inhibits signal transmission from nerve to muscle. The treated muscle relaxes, and mimic-related wrinkle formation diminishes.

The effect is temporary and reversible: the body fully breaks down the effect over the course of months. A targeted, anatomically precise dosage is intended to preserve natural facial expression — the treatment goal is a relaxed, not a “frozen” expression.

Application areas

Which areas are treated

Botulinum toxin type A is suitable for mimic wrinkles in the upper and middle third of the face. Which area is appropriate in your case results from an individual analysis of facial expression. A selection of typical application areas:

Glabella frown lines

The vertical lines between the eyebrows — one of the most common treatment areas.

Also outside aesthetics

Botulinum toxin type A is also used for medical indications — such as for excessive sweating, for nocturnal teeth grinding (bruxism) or as adjunct treatment for certain forms of headache. These applications are discussed separately in a personal consultation.

Procedure

How a treatment proceeds

  1. History and mimic analysis

In conversation, wishes, prior treatments and possible contraindications are clarified. Individual facial expression is assessed — it determines which muscle areas are treated.

  2. The treatment

The active substance is introduced through a very fine needle into precisely defined points. The actual treatment usually takes only a few minutes.

  3. Aftercare instructions

You receive specific guidance for the hours after the treatment — for example regarding sport, heat and body position. Downtime is low.

  4. Onset of effect

A first effect usually becomes visible after a few days, the full result after about two weeks.

  5. Follow-up

If needed, a follow-up review is possible to assess the result and document it for subsequent appointments.

Safety

Tolerability and anaesthesiological background

Botulinum toxin type A is generally well tolerated when applied professionally and anatomically correctly. As with any medical intervention, there are nonetheless possible accompanying effects — usually transient and mild: such as slight pressure sensitivity at the injection sites or small bruises. Rare and usually also transient are effects such as asymmetric facial expression; these can be largely avoided through precise dosage and placement.

Why the anaesthesiological background is relevant

Dr. Ackermann is a specialist with anaesthesiological experience. This results in a focus on treatment that is as low-pain as possible, on the assessment of circulatory reactions and on the confident handling of rare complications. Particularly for patients who approach a treatment with respect, this routine in handling medical situations is a deliberate part of the practice concept.

The treatment is not suitable, among other situations, during pregnancy and breastfeeding or with certain neuromuscular conditions. Possible contraindications are individually checked before each treatment.

Frequently asked questions

What patients often ask

Is the treatment painful?

A very fine needle is used. Most patients describe the brief injections as well tolerable. On request, cooling measures can further increase comfort.

How long does the effect last?

This depends on the indication. A period of about three to six months is usual. After that, the body breaks down the effect, and the treatment can be repeated if desired.

When is the result visible?

A first effect usually becomes visible after a few days, the full result after about two weeks.

Can I work normally afterwards?

As a rule, yes. Downtime is low; a few aftercare instructions for the first hours should be observed.

Can people tell that something has been done?

The treatment goal is a naturally appearing, relaxed expression. A precise, restrained dosage is intended specifically to avoid a “mask-like” look.

Which preparation is used?

Exclusively original preparations approved in Germany with the active substance botulinum toxin type A are used. Which one is suitable specifically depends on indication and area and is explained in conversation.
