Sweaty feet affect daily life in a very concrete way: Closed shoes become uncomfortable, certain floors risky, and the risk of fungal infections increases. Many sufferers change their socks several times a day or avoid certain situations. This is not a matter of sensitivity – it is a medical problem that can be treated.
Plantar hyperhidrosis presents special medical requirements: The stratum corneum of the sole of the foot is so thick that topical anesthetic creams cannot penetrate sufficiently. Those who want to perform the treatment comfortably need precise regional anesthesia – the foot block. The Foot Block: Regional Anesthesia before Botulinum toxin type A Treatment
As a specialist in anesthesiology, I perform an ankle block (foot block at the ankle) before every sole of the foot treatment. This selectively numbs the five nerves that supply the plantar surface:
- Nervus tibialis posterior (medial, behind the medial malleolus)
- Nervus peroneus superficialis (dorsum of the foot, subcutaneously)
- Nervus peroneus profundus (between the extensor tendons)
- Nervus saphenus (medial near the medial malleolus)
- Nervus suralis (lateral between the lateral malleolus and Achilles tendon)
The block takes approximately 10 minutes and lasts 45 to 60 minutes – sufficient for the complete treatment of both soles of the feet. You will not feel any pain during the entire Botulinum toxin type A injection. Only when the anesthesia wears off can you walk normally again.
How Botulinum Toxin Works for Sweaty Feet
Botulinum Toxin Type A specifically inhibits the nerve impulses that stimulate the sweat glands of the sole of the foot to produce sweat. The glands are temporarily inactivated – without affecting the skin itself, without restricting the natural mobility of the feet, and without systemic effects.
- Onset of effect: 3 to 5 days after treatment
- Full result: visible after approx. 2 weeks
- Duration of effect: 3 to 5 months – often longer with regular treatment
- Dosage: 75 to 100 units per sole of the foot – individually adjusted
This is how your treatment works
- Consultation:Medical history, Minor’s test (starch test for sweat measurement), clarification of your situation and treatment goals.
- Perform foot block:Regional anesthesia of both feet at the ankle. Onset time approx. 10 minutes – during this time your feet are completely pain-free.
- Botulinum toxin type A injection:Systematic, close-meshed injection (every 1–1.5 cm) over the entire plantar surface – including toe pads and edges of the feet. Since the feet are completely anesthetized, a more complete coverage is possible than without a block.
- Observation and aftercare:Short resting phase. Walking may be restricted for 2 to 6 hours – please plan for an accompanying person or no driving immediately after the treatment.
