Filler Complications & Second Opinion
Dr. Thomas Ackermann Practice · Herzogenaurach
Filler Complications & Second Opinion –
when something isn’t right.
An asymmetry, a hardening, a result that doesn’t suit your face — or an acute suspicion of a complication after a filler treatment. For all of this, my practice in Herzogenaurach offers a clear point of contact: analysis, ultrasound diagnostics, individual consultation, and if necessary, targeted correction with Hylase.
For acute symptoms: act immediately.
Severe swelling, skin discoloration, or increasing pain after an injection require immediate medical attention. Call the practice or, in an emergency, dial 112.
What I offer:
- Second Opinion: Independent assessment after treatments elsewhere
- Ultrasound Diagnostics: Imaging analysis of filler deposits and complications
- Correction with Hylase: Ultrasound-guided dissolution of unwanted fillers
Description
When a filler treatment does not achieve the desired result
Most hyaluronic acid treatments proceed without complications and deliver the desired result. But not always. Sometimes the result isn’t right — too much volume, an asymmetry, a filler that has shifted. Sometimes hardenings or nodules develop. And in rare cases, more serious complications occur that require quick action.
In my practice in Herzogenaurach, there is a clear point of contact for all these situations. As a specialist in anesthesiology with experience in emergency medicine and ultrasound-guided diagnostics, I am trained and equipped for this spectrum — from a calm second opinion to acute intervention.
Which situations belong here
Dissatisfaction with the result
Asymmetries, overcorrections, filler that doesn’t feel natural, lips that appear too full or too angular, cheeks that distort the face. Everything that can be solved — and in most cases can be solved.
Hardenings and Nodules
Palpable nodules under the skin, hardenings that do not disappear, the so-called “Tyndall effect” (blue discoloration when filler is injected too superficially). These situations require an accurate diagnosis before treatment.
Filler Migration
Especially after lip treatments, hyaluronic acid can migrate away from the injection site over time — above the lip, in the nasolabial fold area. Ultrasound diagnostics makes this visible and enables targeted correction.
Second Opinion
You are unsure whether a result is normal or if something should be corrected. You would like an independent medical assessment before you decide. Without sales pressure, without a preconceived solution.
Acute Complication
Act immediately with these symptoms
The most serious complication in filler medicine is unintentional intravascular injection — when hyaluronic acid enters a blood vessel and interrupts blood flow. This is rare but serious. Typical warning signs:
- Severe, increasing pain after treatment
- White or livid (blue-red) skin discoloration
- Numbness or visual changes
- Swelling that increases significantly more than expected
Call emergency services immediately with these signs: 112
Or contact the practice directly — we are trained and equipped for this situation. Hylase as an antidote and emergency medications are available in the practice.
How a consultation works
Every consultation begins with an open discussion — without evaluating the previous practitioner, without a preconceived solution. My task is exclusively the best assessment and, if necessary, the best correction.
- Anamnesis: What was treated, when, with what — and what is bothering you?
- Ultrasound Diagnostics: Imaging analysis of the affected area. What is still present, where, at what depth?
- Discussion of findings:What is the cause, what are the options — with and without correction?
- Treatment:If correction is advisable:ultrasound-guided hyaluronidase treatmentpossible in the same session.
Why Anesthesiology and Complication Management Belong Together
Complication management is clinical medicine — not aesthetics. It requires knowledge of anatomy, emergency medicine, ultrasound diagnostics, and the safe handling of medication under pressure. As a specialist in anesthesiology with clinical experience at Klinikum Nürnberg, I bring exactly this background. I treat complication cases not despite my aesthetic work — but because both areas share the same medical foundation.
